We are seeking a dedicated and compassionate Housing Support Worker to join our Community Homes for Opportunity (CHO) Program. As a key member of our team, you will work collaboratively with clients to provide individualized and flexible support and wellness planning, helping them maintain their housing, engage with their community, and achieve their goals.

About the Role:

The Housing Support Worker will work in a combination of home and office settings, providing support to clients in their homes and in the community. You will be responsible for:

  • Assisting clients with daily living tasks and activities
  • Developing and implementing individualized support plans
  • Providing advocacy and support to access community resources
  • Collaborating with other team members to ensure comprehensive support
Requirements:

To be successful in this role, you will require:

  • A degree or diploma in health or social services
  • Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Experience working with adults with serious mental illness in the community
  • Knowledge of the Ontario Mental Health Act and mental health reform principles
  • Ability to apply recovery principles and empowerment-oriented philosophies and practices
