Job Summary:

Vancouver Coastal Health is seeking a Community Liaison Worker to join our Integrated Care Team at Heatley Community Health Centre in Vancouver, BC.

The successful candidate will work as part of an integrated health service team, providing support services to clients with mental health and substance use/addiction issues.

Key responsibilities include:

  • Supporting clients with mental health and substance use/addiction issues
  • Working across multiple sites and clinics to provide integrated care
  • Monitoring clients and documenting observations
  • Facilitating community services and resources
  • Liaising with mental health and substance use professionals

Qualifications:

  • Certificate from a recognized Health Care Worker program
  • 2 years' recent experience in a related field
  • Valid driver's license and access to a vehicle
