Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide individualized care within your scope of practice.
  • Use appropriate assessment tools and techniques to assess patients' individual needs and develop care plans.
  • Provide safe and ethical nursing care, continuously monitoring and adjusting care plans as required.
  • Engage patients in their care through a collaborative approach, building rapport and trust.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a RPN or RN in good standing.
  • Valid driver's license and access to a vehicle is required.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility.
  • Willingness to travel to and from patients' residences within a designated area.
