Transplant Care Specialist

We are seeking a skilled Transplant Care Specialist to join our team at Vancouver Coastal Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support in the Solid Organ Transplant Clinic.
Job DescriptionIn this role, you will work closely with our multidisciplinary team to assess clients post-solid organ transplantation, coordinate interdisciplinary assessment and follow-up treatment, and provide education to patients and their families. You will also participate in various internal and external committees as a representative of transplant services and contribute to reviewing literature, analyzing trends, and recommending changes to modify current nursing practices.
Your expertise in nursing practice related to designated patient/client groups, including solid organ transplantation, will enable you to develop and implement effective plans of care, consult with team members, and collaborate with other healthcare providers to address health issues of this client group.
Key Responsibilities- Assess clients post-solid organ transplantation and coordinate interdisciplinary assessment and follow-up treatment.
- Provide education to patients and their families on topics such as infection prevention, rejection management, and self-medication at home.
- Participate in internal and external committees as a representative of transplant services.
- Review literature, analyze trends, and recommend changes to modify current nursing practices.
To be successful in this role, you will need:
- A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Demonstrated ability in nursing practice related to designated patient/client groups, including solid organ transplantation.
- Excellent communication and collaboration skills.
- Ability to work effectively in a fast-paced environment.
