About the Opportunity

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health Authority. As a key member of our Mental Health and Addictions team, you will play a critical role in facilitating access to our services and supporting individuals in navigating the healthcare system.

The Patient Access Navigator will be responsible for:

  • Utilizing screening tools and risk assessments to identify client needs and develop effective care plans
  • Providing brief supportive counseling and psychoeducation to clients and their families
  • Collaborating with community resources and Mental Health and Addictions teams to ensure seamless care transitions
  • Building working relationships with clients and their families to support problem-solving and self-navigation
  • Reducing barriers to treatment and support by identifying client needs and directing them to relevant resources

We are looking for a candidate with a strong background in social work and experience working with individuals with mental health and addiction concerns. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

About You

We are seeking a candidate with:

  • A Bachelor's degree in Social Work
  • Registration with the Nova Scotia College of Social Workers
  • Three years of related experience in Mental Health and Addictions in a healthcare setting
  • Experience working with individuals with mental health and/or substance use/gambling concerns
  • High degree of motivational interviewing skills and engagement techniques
  • Experience utilizing screening and triaging tools
  • Knowledge of harm reduction strategies and stages of change theory
  • Demonstrated awareness of community resources and programs and community development methods
  • Demonstrated knowledge of recovery-oriented practice and patient and family-centered care
  • Non-Violent Crisis Intervention, Mental Health First Aid, or commitment to complete these courses
  • Working knowledge of DSM
  • Ability to navigate and de-escalate situations and offer crisis support using appropriate methods when needed
  • Demonstrated knowledge of cultural sensitivity and cultural competency
  • Competencies in other languages an asset, French preferred
Hours and Compensation

This is a permanent full-time position, 75 hours bi-weekly. The hourly rate is $42.89. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
