Indigenous Social Worker

We are seeking a skilled and compassionate Indigenous Social Worker to join our team at Fraser Health. As a Primary & Community Care Social Worker - Indigenous, you will work closely with clients and their families to provide comprehensive services and therapeutic clinical counseling.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments of Indigenous clients and their families, using a variety of assessment tools to provide client treatment services.
- Provide clinical support to the interdisciplinary team, including physicians, in the development of care plans and services within the assigned area of responsibility.
- Participate on local committees and planning bodies to identify areas to improve the efficiency and effectiveness in service delivery to enhance client care services and reduce service gaps.
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex health care environment, such as a community or home health setting, plus relevant related experience in an indigenous environment/community.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
- Comprehensive knowledge and understanding of counselling and clinical practice models as it relates to indigenous individuals, families, groups and communities.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessment.
- Comprehensive counselling skills, including crisis counseling, brief therapeutic interventions and loss and grief applicable to patients, families and their support networks.
