Job Summary
As an Indigenous Social Worker in our Primary & Community Care team, you will work collaboratively with healthcare professionals to provide culturally sensitive services to Indigenous clients and their families. Your expertise in social work and psychosocial assessments will be pivotal in developing comprehensive care plans that address the unique needs of our Indigenous population.
Key Responsibilities
- Conduct comprehensive social work and psychosocial assessments of Indigenous clients and their families, utilizing various assessment tools to inform treatment plans.
- Collaborate with the interdisciplinary team to develop and implement care plans that prioritize the cultural and social needs of Indigenous clients.
- Provide clinical support to physicians and other healthcare professionals, ensuring seamless care transitions and effective service delivery.
- Participate in local committees and planning bodies to identify opportunities for improving service delivery and reducing service gaps for Indigenous clients.
- Develop and deliver in-services, presentations, and training sessions to promote cultural competence and best practices in Indigenous healthcare.
Requirements
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, with a focus on Indigenous populations.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Comprehensive knowledge of counselling and clinical practice models, as well as cultural sensitivity and awareness.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multicultural environment.
