Primary Care Social Worker

About the Role:
We are seeking a highly skilled and compassionate Primary & Community Care Social Worker - Indigenous - Hope to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive services and therapeutic clinical counseling to clients and their families who identify as Indigenous.
Key Responsibilities:
- Conduct comprehensive social work and psychosocial assessments of Indigenous clients and their families using a variety of assessment tools.
- Provide clinical support to the interdisciplinary team, including physicians, in the development of care plans and services within the assigned area of responsibility.
- Participate on local committees and planning bodies to identify areas to improve the efficiency and effectiveness in service delivery to enhance client care services and reduce service gaps.
- Collaborate with the interdisciplinary team in client care planning, including interviewing clients and/or families to assess and determine care plans and type of service required.
- Support a seamless approach to care and service delivery across the healthcare system by participating in the admission and discharge process of clients in acute care and community.
Qualifications:
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex health care environment such as a community or home health setting, plus relevant related experience in an indigenous environment/community.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
