Job Summary:

The University Health Network is seeking a skilled Patient Educator Diabetes to join our team. As a member of the Diabetes/Endocrine Program multi-disciplinary team, you will provide patient education in diabetes for both clinic outpatients and in-patients preparing for discharge home.

Main Responsibilities:

- Teach patient education in diabetes for both clinic outpatients and in-patients preparing for discharge home

- Provide patient education on medication self-administration by injection and insulin pump

- Teach patient self-measurement of glucose levels using continuous and intermittent methods of monitoring

- Adjustment of Insulin and oral hypoglycemic agents using a Medical Directive

Qualifications:

- Current Registration with the College of Nurses of Ontario required

- Basic Cardiac Life Support Certification required

- Flexible and able to work effectively and efficiently within a interdisciplinary patient care team

- Must be able to meet the physical requirements of the position

- Excellent interpersonal, communication and decision making skills

- Excellent organizational and time management skills

- Excellent computer skills
