We are seeking a highly skilled Rehabilitation Case Manager to join our team at St. Paul Abilities Network.

About the Role

This is a full-time position that requires the ability to work independently and under pressure. The successful candidate will be responsible for providing support services to individuals with disabilities, including assessing needs, developing rehabilitation plans, and monitoring progress.

Responsibilities
  • Client Assessment: Conduct comprehensive assessments of clients' physical and emotional needs, identifying areas for improvement and developing personalized rehabilitation plans.
  • Case Management: Work closely with clients, family members, and healthcare professionals to coordinate care, provide guidance, and ensure seamless transitions between services.
  • Community Outreach: Establish and maintain relationships with community organizations, healthcare providers, and other stakeholders to promote access to resources and services.
  • Documentation: Maintain accurate and detailed records of client interactions, progress, and outcomes, using industry-standard software and tools.
Requirements

To be successful in this role, you must have:

  • A degree in a related field (e.g., social work, psychology, occupational therapy), or equivalent experience;
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals and groups;
  • Strong problem-solving and analytical skills, with the ability to think critically and creatively;
  • The ability to work independently and as part of a team, prioritizing tasks and managing competing demands;
  • Proficiency in industry-standard software and tools, with a willingness to learn new technologies and systems.
What We Offer
