As a key member of our interprofessional team, you will be responsible for delivering high-quality patient care, providing clinical leadership, and promoting a culture of excellence.

Key Responsibilities
  • Primary Health Care Delivery: Provide comprehensive health services to residents in LTC, including the management of acute and chronic medical conditions, therapeutic management, health promotion, and disease/injury prevention.
  • On-call Coverage: Provide on-call coverage for duties and schedules as required, ensuring seamless patient care.
  • Clinical Leadership: Provide clinical leadership to LTC team members, centered around resident care needs, including resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behavior management.
  • Health Assessments: Conduct health assessments using various tools and techniques based on resident needs, including physical, psychosocial, emotional, cultural, and ethnic dimensions of health.
  • Diagnostic Investigations: Perform physical examinations, identify and interpret normal and abnormal findings, and order diagnostic investigations and interpret results using evidence-based clinical reasoning.
  • Person-centered Care Plans: Collaborate with residents and family members to develop person-centered care plans, synthesizing health information using critical inquiry and clinical reasoning to formulate a diagnosis, identify health risks, and states of health/illness.
  • Therapeutic Management: Select appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological, and mental stability, achieving optimal health.
  • Safe Resident Care: Promote safe resident care by mitigating harm and addressing immediate risks for residents and others affected by adverse events and near misses.
  • Discharge Planning: Liaise with discharge planning services for hospitalized residents to ensure a smooth and safe transition back to the LTC home.
  • Quality Improvement: Collaborate with leaders and community health services in initiatives for the development, implementation, and evaluation of systems to promote continuity of resident care across the continuum and participate in quality improvement and research initiatives.
  • Continuing Competence: Demonstrate active participation in the CNO Quality Assurance (QA) Program to ensure maintenance of continuing competence to practice.
  • Time Allocation: Dedicate 80% of the NP's time towards direct care to residents, and 20% to research and providing education to improve resident care.
  • Cultural Competence: Ability to incorporate knowledge of diversity, cultural safety, and the determinants of health in the assessment, diagnosis, and therapeutic management of residents and the evaluation of outcomes.
  • Leadership and Collaboration: Leadership skills, working well within an inter-professional team, ability to foster effective working relationships, and ability to provide clinical supervision, education, and mentoring for nursing students, medical students, and other learners.
  • Communication: Strong and effective verbal and written communication skills.
  • Flexibility: Ability to work with flexibility for potentially evenings or weekends, participates in providing after-hours and on-call coverage.
  • Regulatory Knowledge: Knowledge of RNAO Best Practice guidelines and MLTC Regulations and Act.
Requirements
  • Registration: College of Nurses of Ontario registration in the Extended Class.
  • Education: Bachelor's degree in Nursing (BScN); Master's degree in Nursing (MSN) with a Nurse Practitioner specialization preferred.
  • Membership: Membership with Nurse Practitioners' Association of Ontario (NPAO) (preferred).
  • Liability Insurance: Has acquired professional liability insurance (with either RNAO or other liability insurance).
  • Certification: National certification in gerontology is an asset.
  • Skills: Ability to manage crises in an effectively, professional manner, excellent written skills and report writing for a clinical setting, ability to communicate with clients in a non-judgmental manner, practicing active listening skills to understand client needs, and coaching of clients as collaborative partners in their health care.
  • Certifications: Basic CPR certification required.
  • Screening: Must show evidence of valid current Vulnerable Sector Check (VSC).
  • Immunizations: Other immunizations as mandated by the Ministry of Long-term Care (MoLTC).
