Job Summary
RhynoCare is seeking a skilled Nurse Practitioner to join our team in a Long-term Care setting. As a Travel Nurse Practitioner, you will provide primary health care to residents, managing acute and chronic medical conditions, and promoting health and wellness.
Key Responsibilities
- Deliver comprehensive health services to residents, including therapeutic management, health promotion, and disease prevention.
- Provide on-call coverage and participate in after-hours and on-call rotations.
- Conduct health assessments, identify health risks, and develop person-centered care plans.
- Collaborate with residents, families, and healthcare teams to ensure seamless care transitions.
- Participate in quality improvement initiatives and contribute to the development of systems to promote continuity of care.
- Lead and support educational programs for the care team, including professional advisory and research initiatives.
Requirements
- College of Nurses of Ontario registration in the Extended Class.
- Bachelor's degree in Nursing (BScN); Master's degree in Nursing (MSN) with a Nurse Practitioner specialization preferred.
- Membership with Nurse Practitioners' Association of Ontario (NPAO) (preferred).
- Valid current Vulnerable Sector Check (VSC) and other immunizations as mandated by the Ministry of Long-term Care (MoLTC).
