Job Summary

We are seeking an experienced Occupational Therapist to join our team as a Home Care Specialist. As a key member of our team, you will be responsible for providing therapy services to clients in their homes, assessing their needs, and developing personalized care plans to promote their independence and well-being.

Key Responsibilities

  • Assess clients' needs and develop comprehensive care plans to promote their independence and well-being.
  • Provide therapy services to clients in their homes, including occupational therapy, health promotion, and equipment prescription.
  • Collaborate with clients' family members, caregivers, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements
