Patient Focused Care Responsibilities

  • Determine the appropriateness of drug therapy for patients
  • Monitor medication therapy for effectiveness, adverse reactions, and interactions
  • Counsel patients regarding prescriptions, OTC products, and health-related issues
  • Answer questions from patients that require therapeutic knowledge or clinical analysis
  • Provide clinical services as appropriate for eligible patients
  • Administer injections/immunizations as needed
  • Promote health and wellness by educating patients on disease prevention and lifestyle modifications
  • Collaborate with team members and other healthcare professionals to optimize patient care outcomes
  • Supervise pharmacy students or provisional pharmacists in restricted activities
  • Adhere to federal, provincial, and territorial regulations governing pharmacy practice

Education and Experience
