Patient Health Services Coordinator

Job Summary:
SEA MAR Community Health Centers is seeking a Patient Health Navigator to join our team through our AmeriCorps Program based in Seattle. The AmeriCorps Team Member will work in Vancouver, Washington, through the host clinic.
Key Responsibilities:
- Schedule appointments and assist patients in navigating health system barriers
- Initiate communication with patients to address their needs and make referrals to relevant services
- Participate in mandatory trainings, service projects, and team building activities throughout the 10-11 month term of service
- Create informational materials and resources for patients, including posters, promotional programs, and resource binders
- Develop a community resource guide to share with patients and the AmeriCorps-SEA MAR group
- Review electronic health record charts to identify patients in need of health screenings and services, and schedule appropriate appointments
- Collaborate with SEA MAR medical clinic staff and quality improvement staff to identify patients in need of screenings and to inform care teams about preventative care
- Work with SEA MAR staff to implement additional preventive health screening and recall efforts
- Participate in an AmeriCorps site health project, including community outreach, health interventions, and health events
- Assist in increasing public awareness about health coverage enrollment opportunities through community outreach and events
- Develop and distribute informational materials and establish referral relationships with local organizations and agencies
Requirements:
- Be at least 17 years old at the commencement of service, or be an out-of-school youth 16 years old with special circumstances
- Have a high school diploma or GED, or agree to obtain prior to using the education award
- Be a citizen, national, or lawful permanent resident of the United States
- Have excellent verbal and written communication skills, including the ability to interpret and utilize information, and to communicate effectively with people of all backgrounds and educational levels
- Be able to work with minimal supervision, use good judgment, meet deadlines, and prioritize workloads
- Be able to maintain confidentiality and professionalism in all interactions
- Be computer literate and have good mathematical skills, with experience in Windows Office programs preferred
