Job Summary:The Patient Health Navigator will work closely with patients, healthcare providers, and community organizations to identify and address barriers to healthcare access. This includes scheduling appointments, facilitating communication between patients and healthcare providers, and providing education and support to patients and their families.
Key Responsibilities:- Schedule appointments and navigate patients through the healthcare system
- Initiate communication with patients and assist with support in making referrals to address patient needs
- Participate in mandatory trainings, service projects, and team-building activities
- Create informational materials and resources to support patient education and engagement
- Develop and maintain relationships with community organizations and healthcare providers to enhance patient care
- Review electronic health records and identify patients in need of important health screenings and services
- Collaborate with Sea Mar medical clinic staff and quality improvement staff to inform care teams about preventative care and reach out to patients in need
- Participate in community outreach and health events to promote health education and awareness
- Be at least 17 years of age at the commencement of service
- Have a high school diploma or GED or agree to obtain prior to using the education award
- Be a citizen, national, or lawful permanent resident of the United States
- Have excellent verbal and written communication skills, including ability to compile, interpret, and utilize information
- Be able to work with minimal supervision, use good judgment, meet deadlines, and prioritize workloads
- Be able to maintain confidentiality and professionalism in all interactions
- Be computer literate and have good mathematical skills
- Have reliable transportation
