Job Summary

We are seeking a Patient Care Coordinator to join our team at PHSA. As a Patient Care Coordinator, you will be responsible for providing high-quality patient care and administrative support to our patients and families.

Key Responsibilities

  • Answer telephone calls, take messages, and relay information to appropriate personnel
  • Enter patient information into the Cerner system for discharge, transfer to other facilities, and transfer between units
  • Make arrangements for patients requiring procedures, both inside and outside the facility
  • Transcribe and process physicians' orders to appropriate forms, medication records, and laboratory tests
  • Check charts for unprocessed orders, process them as necessary, and notify the charge nurse
  • Prepare charts for new admissions, including making identification labels and imprinting chart sheets
  • Maintain patient records, including filing loose reports and retrieving previous charts from Health Records
  • Prepare charts for patient transfers to other facilities
  • Complete demographic information on forms for Vital Statistics
  • Notify departments of patient requirements, including discharge or transfer

Requirements

  • Grade 12, medical terminology, and graduation from an approved Nursing Unit Clerk program, plus one year's recent related experience
  • Ability to communicate effectively, both verbally and in writing
  • Ability to deal with others effectively
  • Ability to utilize computer software applications
  • Ability to keyboard 40 wpm
  • Ability to organize work
  • Physical ability to carry out the duties of the position
