Job Summary

We are seeking a skilled and dedicated Primary Care Nurse Practitioner to join our team at Ontario Health Clinics Brantford FHO. This is a full-time, permanent position that offers a unique opportunity to make a positive impact on the health and well-being of our patients.

About the Role
  • Provide comprehensive primary health care to patients, including assessing, diagnosing, and evaluating acute and chronic illnesses, prescribing treatments and medications, and performing follow-up care.
  • Perform routine tests such as PAP tests, administer immunizations, and provide health education to promote healthy lifestyles.
  • Develop and implement patient care plans, collaborating with other healthcare providers as needed.
  • Use electronic medical records to document patient information and maintain accurate records.
Requirements
  • A minimum of [insert estimated salary] per year, commensurate with experience.
  • Previous experience in primary care is strongly preferred, but not required.
  • Excellent communication, critical thinking, and interpersonal skills are essential for success in this role.
