Continuing Care Coordinator

We are seeking a skilled and compassionate Continuing Care Coordinator to join our team at Nova Scotia Health and IWK Health. As a key member of our healthcare team, you will play a vital role in assessing, planning, and coordinating care for clients referred to our Continuing Care services.
The ideal candidate will have a strong background in healthcare, with experience in case management, discharge planning, and community-based care. You will be responsible for conducting thorough assessments, identifying client needs, and developing personalized care plans that promote optimal health outcomes.
As a Continuing Care Coordinator, you will work collaboratively with interprofessional teams, including healthcare providers, social workers, and community agencies, to ensure seamless care transitions and effective resource allocation.
You will be responsible for:
- Conducting comprehensive assessments of clients' physical, emotional, and social needs
- Developing and implementing individualized care plans that address client goals and priorities
- Coordinating and authorizing services and resources to support client care
- Facilitating communication and collaboration among healthcare providers, family members, and caregivers
- Monitoring and evaluating client progress, making adjustments to care plans as needed

About YouWe are looking for a candidate with:
- A Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience in a healthcare setting, with a focus on case management, discharge planning, or community-based care
- Excellent assessment, interviewing, and negotiation skills
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Valid driver's license and access to a reliable vehicle
This is a permanent full-time position, with a 70-hour bi-weekly schedule. You will be compensated at a rate of $46.75 per hour, with opportunities for professional growth and development.
