Continuing Care Coordinator

We are seeking a skilled and compassionate Continuing Care Coordinator to join our team at Nova Scotia Health Authority. As a key member of our healthcare team, you will play a vital role in providing high-quality care and support to individuals and families in our community.
Key Responsibilities:
- Assess and develop care plans for clients referred to Continuing Care, taking into account their unique needs and goals.
- Authorize services and make referrals to other professional and volunteer resources to support clients in their homes and communities.
- Work collaboratively with interprofessional teams to facilitate hospital discharge and ensure a smooth transition to community-based care.
- Provide education and support to clients, families, and caregivers to empower them to make informed decisions about their care.
- Maintain ongoing communication with clients, families, caregivers, and healthcare teams to ensure a coordinated and person-centered approach to care.
Requirements:
- Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
- Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body.
- Minimum three years' experience in a community or long-term care setting.
- Demonstrated knowledge and/or experience in dedicated case management or discharge planning.
- Competent with computer use, keyboarding, and able to conduct assessments using a laptop.
- Knowledge of community agencies and resources.
- Excellent assessment, interviewing, and negotiation skills.
- Excellent verbal and written communication skills.
- Ability to work independently and co-operatively in a busy multidisciplinary situation.
- Ability to multi-task, prioritize workload, and manage time efficiently.
- Valid driver's license and access to a reliable vehicle.
- Competencies in other languages preferred; French an asset.
