The Nova Scotia Health Authority is seeking a skilled Care Management Professional to join our team in the Continuing Care department. As a key member of our interprofessional team, you will be responsible for assessing the needs of clients referred to Continuing Care, developing and implementing care plans, and authorizing services to support clients in their homes and communities.
This is a challenging and rewarding role that requires strong assessment, communication, and problem-solving skills. You will work closely with clients, families, caregivers, and healthcare teams to identify goals, unmet needs, and resource requirements, and develop plans to meet those needs. You will also be responsible for authorizing services, making referrals, and linking clients with community resources to support their care.
The ideal candidate will have a strong background in healthcare, preferably in a community or long-term care setting, and experience with case management or discharge planning. You will be a skilled communicator, able to work effectively with clients, families, and healthcare teams, and have a strong understanding of community resources and services. You will also be able to work independently and as part of a team, and have strong time management and organizational skills.

To be successful in this role, you will need:
- A Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience in a healthcare setting
- Demonstrated knowledge and experience in community or long-term care settings
- Knowledge and experience in case management or discharge planning
- Excellent assessment, interviewing, and negotiation skills
- Excellent verbal and written communication skills
- Ability to work independently and as part of a team
- Ability to prioritize tasks and manage multiple responsibilities
This is a permanent full-time position, working 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
