About the Role:

The Southeast Child Family Services is seeking a skilled Transitional Worker to join our team. As a Transitional Worker, you will play a vital role in fostering a positive and healthy environment for young adults in our care.

Responsibilities:

  • Support young adults in developing and achieving their personal goals and life plans, providing wrap-around services as needed.
  • Assist with accessing essential resources, such as food, clothing, and medical care, as well as facilitating connections with therapy, legal support, and community services.
  • Ensure the cleanliness and organization of transitional housing facilities, promoting a conducive living environment.
  • Organize outings and accompany young adults to appointments, providing case management support as required.
  • Perform other duties as assigned by the Agency.

Qualifications:

  • Post-secondary degree or diploma in social services, or equivalent combination of education and experience.
  • Knowledge of relevant legislation, including The Child and Family Services Act and Indigenous child welfare practices.
  • Demonstrated understanding and appreciation of Anishinaabe cultural beliefs and values.
  • Lived experience with addiction and recovery considered an asset.
  • Experience providing supports and interventions for trauma, crisis, and other issues.
  • Proficiency in Microsoft Office and excellent communication skills.
  • Valid driver's license and reliable vehicle.
  • Satisfactory background checks and ability to travel to designated communities.
