Job Summary

The Renal Patient Transition Coordinator plays a vital role in facilitating the movement and transition of renal patients between healthcare facilities and renal units. This position requires strong communication and interpersonal skills, as well as the ability to work independently and collaboratively with patients, families, and healthcare teams.

Key Responsibilities

  • Facilitate patient transfers between healthcare facilities and renal units.
  • Coordinate the movement and transition of renal patients between their communities and homes and various healthcare facilities and agencies.
  • Participate in assessment and decision-making processes to ensure effective patient movement and optimal utilization of resources.
  • Liaise with various programs, agencies, and facilities to coordinate patient movement.
  • Develop guidelines, protocols, and educational strategies related to patient movement and transition.
  • Provide dialysis therapy information and education to patients, families, and healthcare providers.
  • Provide support and counseling to renal patients and families.
  • Assist patients in making informed choices regarding their renal replacement therapies.
  • Provide clinical support to the eKHR Team and assist with education to provincial Renal Program interdisciplinary teams.

Requirements

  • Four years of related experience in renal health, peritoneal dialysis, and hemodialysis.
  • Knowledge and working experience with electronic health records used in the patient transfer process.
  • Experience and training in motivational counseling is an asset.
  • Bachelor of Nursing required.
  • Completion of Manitoba Nephrology Nursing Course.
  • Completion of Manitoba Indigenous Cultural Safety Training (MISCT) required.
  • CRNM registration required.
  • Valid driver's license and a vehicle.

Qualifications and Skills

  • Superior communication and interpersonal skills.
  • Ability to work independently and collaboratively with patients, families, and healthcare teams.
  • Ability to facilitate patient transfers between healthcare facilities and renal units.
  • Ability to establish and maintain effective working relationships.
  • Ability to establish and maintain effective resource contacts.
  • Knowledge and experience with computers and proficiency in Microsoft Office software.
  • Subject to a criminal records check.

Physical Requirements

  • Ability to work flexible hours.
  • Ability to travel to patients' homes and various healthcare and long-term care facilities.
