Job Title: Renal Patient Transition Coordinator

Shared Health is seeking a highly skilled and dedicated individual to fill the role of Renal Patient Transition Coordinator. This position plays a critical role in facilitating the movement and transition of renal patients between healthcare facilities and renal units, ensuring seamless care and optimal utilization of resources.

Key Responsibilities:
  • Coordinate patient transfers between healthcare facilities and renal units, ensuring timely and effective movement.
  • Collaborate with interdisciplinary teams to assess patient needs and develop effective transition plans.
  • Liaise with healthcare providers, patients, and families to ensure smooth transitions and optimal care.
  • Develop and maintain relationships with healthcare facilities, renal units, and community partners to ensure effective communication and coordination.
  • Provide education and support to patients and families on renal replacement therapies and healthcare options.
  • Assist in the development of guidelines, protocols, and educational strategies related to patient movement and transition.
Requirements:
  • Four years of related experience in renal health, peritoneal dialysis, and hemodialysis.
  • Knowledge and working experience with electronic health records used in patient transfer processes.
  • Experience and training in motivational counseling an asset.
  • Bachelor of Nursing required, with completion of Manitoba Nephrology Nursing Course and Manitoba Indigenous Cultural Safety Training (MISCT) required.
  • Canadian Nurses Association Certification – Nephrology (CNEPH) preferred.
  • CRNM registration required, with valid driver's license and vehicle.
Qualifications and Skills:
  • Superior communication and interpersonal skills.
  • Demonstrated ability to work independently and collaboratively with patients, families, and interdisciplinary teams.
  • Ability to establish and maintain effective working relationships and resource contacts.
  • Knowledge and experience with computers and proficiency in Microsoft Office software.
  • Demonstrated knowledge and skills in research preferred.
Physical Requirements:
  • Ability to work flexible hours and travel to patients' homes and healthcare facilities.
