About the Role:

The Clinical Nursing Supervisor is a key member of our team, responsible for supervising nurses who provide high-quality care to clients in the community. This role requires a strong clinical background, excellent leadership skills, and the ability to work collaboratively with other healthcare professionals.

Key Responsibilities:

  • Supervise and support nurses to ensure they are providing safe and effective care to clients.
  • Respond to concerns from clients, families, and other team members, and provide guidance and support as needed.
  • Provide ongoing leadership and coaching to nurses, including regular meetings and performance evaluations.
  • Initiate investigations into patient safety incidents and document findings to ensure timely resolution and action plans.
  • Work collaboratively with the nursing team to ensure accurate and timely clinical documentation and submission of reports.
  • Engage clients in problem-solving and health teaching, building rapport and trust to identify their healthcare needs, strengths, capacities, and goals.
  • Engage staff in problem-solving, offering education, coaching, and mentoring to resolve issues.
  • Maintain knowledge, skills, and attitudes to provide safe, efficient, and effective service in all areas of practice, staying up-to-date with changes in the practice setting that affect scope of practice.

Requirements:

  • Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
  • Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
  • Palliative experience an asset.
  • Comprehensive patient health assessment skills required.
  • Understanding of the principles of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines required.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in clinical practice relevant to home health care.
  • Effective critical thinking and problem-solving skills.
  • Valid driver's license and access to reliable transportation.
  • Able to provide a current, clear Vulnerable Sector Screening (VSS).
  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) considered an asset.
