Job Summary:

We are seeking an experienced Senior Diabetes Nurse Educator to join our team at Royal University Hospital in Saskatoon, Saskatchewan. As a key member of our Chronic Disease Management team, you will be responsible for providing high-quality education and support to patients with diabetes and their families.

About the Role:

This is a full-time regular position, requiring a valid Class 5 driver's license and a vehicle available for work. The successful candidate will have a strong background in adult education and experience working with complex patients and families in an outpatient setting.

Responsibilities:

  • Develop and implement individualized education plans for patients with diabetes
  • Conduct group education classes for outpatients in a variety of settings
  • Liaise with families, physicians, dietitians, staff nurses, and community agencies to provide comprehensive care
  • Participate in departmental/research activities related to program development, planning, quality improvement, and evaluation
  • Implement program/resource development related to needs assessment, planning, program design, implementation, and evaluation

Requirements:

  • 3 years related clinical nursing experience with patients and families with diabetes disease management required
  • 1 year experience in adult education and group facilitation related to cardiovascular and diabetes disease management required
  • Valid Class 5 driver's license and reliable vehicle available for work
  • Master's degree in nursing or related field an asset
