Primary Care Nurse

We are seeking a highly skilled Registered Nurse to join our team at Kitimat General Hospital. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement formal education programs for patients, families, and the general public.
- Collaborate with the interprofessional team to provide comprehensive care and services to individuals, groups, and communities.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
