Primary Care Team Leader

We are seeking a highly skilled and experienced Primary Care Team Leader to join our team at Northern Health Authority. As a key member of our Primary Care department, you will be responsible for providing leadership and guidance to our interdisciplinary teams, ensuring the delivery of high-quality patient care and services.
Key Responsibilities- Provide leadership and supervision to Primary Care teams, ensuring the delivery of high-quality patient care and services.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to develop and implement care plans.
- Supervise and mentor staff, providing guidance and support to ensure the achievement of team goals and objectives.
- Develop and implement policies and procedures to ensure compliance with regulatory requirements and Northern Health Authority standards.
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and services.
- Baccalaureate degree in a social or behavioral science field relevant to the position.
- Three to five years of recent related experience, including experience in individual, family, and group modalities of practice in the clinical specialty of the assigned area.
- Valid BC Driver's License.
