About the Role

We are seeking a highly skilled Cultivating Community Rehabilitation Specialist to join our team at BC Mental Health & Substance Use Services in Coquitlam, BC.

Job Summary

This role involves providing rehabilitation services to individuals with complex mental health and substance use challenges. The successful candidate will work collaboratively with inter-professional teams to develop and implement individualized rehabilitative treatment plans.

Key Responsibilities

  • Conduct comprehensive assessments to identify patient strengths, limitations, and functional levels for program involvement.
  • Develop and implement psychosocial rehabilitative programs to address patient needs, including group-based rehabilitation and vocational rehab programs.
  • Collaborate with the inter-professional team to provide recommendations and participate in the development of individualized rehabilitative treatment plans.
  • Maintain accurate records of patient progress and modify treatment plans as necessary.
  • Work collaboratively with other rehabilitation colleagues and treatment team members to analyze patient needs and develop new program initiatives.

Requirements

  • Graduation from a recognized recreational, rehabilitation or related health or human services program, plus two years recent related experience.
  • Valid Food Safe Certificate.
  • Valid Class 4 BC Driver's License.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
