Strategic Care Director

We are seeking a skilled and experienced professional to fill the Director role in our Continuing Care department.
Reporting to the Senior Director, Continuing Care, the successful candidate will be accountable for the overall planning, development, implementation, and evaluation of Continuing Care services and programs, as well as stewardship of all resources allocated to continuing care at the zone level.
Key Responsibilities- Lead the Continuing Care leadership team at the zone level, providing strategic and operational direction to continuing care employees.
- Collaborate with stakeholders across all levels of the health system to ensure effective service delivery.
- Ensure services are evidence-informed and consistent with best practices while ensuring resource sustainability.
- Develop and execute plans based on evidence, with commitment to quality improvement and enhanced customer service.
- Baccalaureate degree in a related field and a minimum of 7 years recent and related leadership/management experience in a healthcare field.
- Demonstrated leadership skills in a healthcare environment.
- Demonstrated commitment to a philosophy of independence for those who require care and to the utilization of evidence to inform practice.
- Strong human resource management, financial management, interpersonal, and communication skills.
- A competitive hourly rate of $53.91 - $71.63.
