Job Summary

The Foundation Health Centre is seeking a highly skilled and compassionate Social Worker to join our multidisciplinary team. As a Licensed Clinical Social Worker, you will play a vital role in providing therapy services to individuals with complex mental health illnesses and medical conditions.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of clients, utilizing evidence-based approaches, to identify mental health and wellness issues.
  • Therapy Services: Develop and implement outcome-oriented therapy plans, collaborating with clients, caregivers, and healthcare providers to ensure effective treatment.
  • Caseload Management: Independently manage a complex caseload, prioritizing client needs and providing education and counseling on functional and rehabilitation issues.
  • Inter-Agency Collaboration: Participate in inter-agency and multidisciplinary team case management coordination, ensuring seamless care delivery.
  • Communication and Education: Provide information and guidance to caregivers, healthcare providers, staff, and community agencies, promoting effective communication and collaboration.
Requirements
  • Education: Master's degree in Social Work (M.S.W.) preferred; Bachelor's degree in Social Work with relevant experience also considered.
  • Registration: Current full registration with the New Brunswick Association of Social Workers (NBASW).
  • Experience: Recent, related experience working in a psychiatric or counselling setting with individuals who have complex mental health issues or equivalent.
