Job Summary

Foundation Health Centre is seeking a skilled Social Worker to join our multidisciplinary healthcare team. As a Licensed Clinical Social Worker, you will provide therapy services to individuals with complex mental health illnesses and medical conditions, enabling them to achieve optimal health outcomes.

Key Responsibilities
  • Assess clients using an evidence-based approach, reviewing records, gathering information, and determining suitability of services to address mental health and wellness issues.
  • Plan outcome-oriented therapy services by collaborating with clients, caregivers, and other healthcare providers.
  • Establish and prioritize expected outcomes, discuss service implementation options, and determine sources and duration of service to meet identified short and long-term client goals.
  • Manage a complex caseload using knowledge, skills, and expertise in professional practice.
  • Participate in inter-agency and multidisciplinary team case management coordination as required.
  • Provide education and counseling to clients and families regarding functional and rehabilitation issues.
  • Offer information and guidance to other caregivers, health service providers, staff, and community agencies as needed.
Requirements
  • Masters of Social Work degree (M.S.W.) preferred; Bachelor's Degree in Social Work with relevant work experience will also be considered.
  • Current full registration with the New Brunswick Association of Social Workers (NBASW).
  • Recent, related experience working in a psychiatric or counselling setting with individuals who have complex mental health issues or equivalent.
