Social Worker I

Fraser Health Authority is seeking a skilled Social Worker I to join our team at Burnaby Hospital. As a key member of our interdisciplinary team, you will provide psychosocial assessments, identify patient/family issues, and develop treatment plans to support our patients' well-being.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient/family needs and develop individualized treatment plans.
- Provide clinical and supportive counseling services to patients and families, addressing issues related to illness, adjustment, and lifestyle changes.
- Collaborate with healthcare teams to provide crisis intervention, crisis resolution, and support services.
- Connect patients and families with community and rehabilitation resources, facilitating access to financial, vocational, transportation, childcare, and accommodation support.
- Deliver educational services to individuals, couples, families, or groups, addressing the psychosocial implications of treatment modalities and lifestyle changes.
- Participate in quality improvement activities, contributing to the development, implementation, and evaluation of departmental and program goals, objectives, policies, and procedures.
- Prepare and maintain required statistics and reports.
Bachelor's degree in Social Work from an approved school of Social Work, plus three (3) years' recent, related clinical experience.
Current full registration with the BC College of Social Workers.
