Vancouver Coastal Health is seeking a skilled Licensed Practical Nurse (LPN) to fill the role of Home Support Supervisor in Squamish, British Columbia.
Key Responsibilities- Assess and develop care plans for clients with acute, chronic, and palliative conditions, working collaboratively with healthcare teams and clients to achieve safe and effective care.
- Provide leadership and coaching to Home Support Workers, promoting a team-based environment and ensuring high-quality care delivery.
- Collaborate with primary care providers and other healthcare professionals to coordinate care and services for clients.
- Conduct assessments of service eligibility and facilitate the development, implementation, and adaptation of care plans.
- Deliver direct care and provide care management to assist clients in managing their own care and navigating community services.
- Graduate of a recognized program for Licensed Practical Nurses, with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years of recent experience working with geriatric clients, adults with chronic and acute illness.
- One year of experience in a supervisory role.
- Valid BC Class 5 Driver's License, with local area travel requiring the use of a personal vehicle.
- Comprehensive knowledge of healthcare disciplines and their role in client and family care.
- Demonstrated medication administration skills as outlined in the LPN Scope of Practice.
- Ability to provide leadership and coach team members to promote an efficient and effective team-based environment.
- Ability to analyze and resolve problems efficiently and be resourceful.
- Ability to adjust communication to suit the recipient and/or situation within a complex community environment.
