Humber River Health, a leader in innovative healthcare, is seeking a highly skilled Mental Health Nurse Specialist to join our dynamic team.

About the Role

We are looking for a compassionate and experienced Registered Nurse to work in our Outpatient Mental Health Program. As a Mental Health Nurse Specialist, you will be part of a multidisciplinary team that provides high-quality care to individuals with mental health needs.

Key Responsibilities
  • Process and review referrals to ensure timely access to services
  • Liaise with referral sources to gather necessary information
  • Conduct thorough intake assessments to identify patient needs
  • Develop and implement individualized treatment plans
  • Provide education and support to patients and families
  • Collaborate with internal teams to ensure seamless service delivery
Requirements
  • Current registration with the College of Nurses of Ontario
  • BCLS/CPR certification required
  • Experience working in mental health or addictions preferred
  • Demonstrated ability to provide individualized care and support
  • Excellent communication and interpersonal skills
What We Offer
