Healthcare Support Specialist

We are seeking a skilled Healthcare Support Specialist to join our Transitions team at SE Health. This exciting opportunity allows you to be at the forefront of healthcare innovation in Canada, providing care that is truly client-centered.
Job SummaryAs a Healthcare Support Specialist, you will work as part of an interdisciplinary team to provide comprehensive care to clients in their homes and communities. You will assist clients with daily activities, maintain effective relationships and communication with other disciplines, and participate in regular team meetings.
Responsibilities- Provide continuous and comprehensive support to your team in the delivery of care for clients
- Maintain effective relationships/communication and works collaboratively with other disciplines to implement interdisciplinary care plans within your role
- Participate in regular interdisciplinary team communication (e.g. huddle, rounds, status updates)
- Use communication tools to effectively communicate changes in the client's condition to the primary care lead/clinician and other members of the care team as required
- Assist clients in care activities as assigned by the Physiotherapist and/or Occupational Therapist
To be successful in this role, you will need:
- An OTA/PTA diploma or equivalent from an accredited post-secondary program
- A minimum one year of relevant work experience is preferred
- Experience in one of the following areas: home/community care, acute care, chronic disease management, health promotion and palliative care

Salary InformationThe estimated salary for this role is $65,000 - $80,000 per year, depending on experience and qualifications.
