Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
  • Provide individual, group, and family therapy to address a range of social and emotional issues.
  • Collaborate with healthcare teams to coordinate patient care and ensure seamless transitions between services.
  • Develop and maintain relationships with community resources to support patient needs.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Contribute to program planning, evaluation, and research to inform best practices in social work.

Requirements:

  • Completion of a Bachelor's degree in Social Work.
  • Active or eligible registration with the Alberta College of Social Workers (ACSW).
  • Strong interpersonal, communication, and teamwork skills.
  • Proficiency in individual and family assessment/intervention.
  • Understanding of ethics and ethical decision-making.
  • Demonstrated familiarity with community resources.

Preferred Qualifications:

  • Counseling and mental health experience.
  • Housing and family support work experience.
  • Healthcare and hospital experience.
  • Experience in a rehabilitation setting.
