Care Coordinator for Indigenous Health Services

Job DescriptionThe Care Coordinator will play a critical role in coordinating care for Indigenous patients, ensuring that they receive comprehensive and culturally safe services. This includes:
- Validating demographic data and health information for Indigenous patients;
- Covering complex care coordination for patients, including forwarding necessary information to community health centres and agencies;
- Communicating with healthcare professionals and community agencies to ensure seamless transitions of care;
- Maintaining up-to-date knowledge of Indigenous health policies and direction;
- Providing consultation and education to staff on Indigenous cultures, social determinants of health, and health beliefs;
- Facilitating linkages to community programs and services;

RequirementsTo be successful in this role, you will need:
- A graduate degree in nursing or a related field;
- Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM);
- At least two years of experience in women and/or child-centred practice;
- Strong knowledge of Indigenous history, cultures, communities, and health policy;
- Excellent communication and interpersonal skills;
- Ability to work independently and as part of a team;
- Physical ability to perform the duties of the position.
