Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Mental Health team at North York General Hospital. As a key member of our interdisciplinary team, you will provide patient-centered care to individuals and families navigating mental health challenges.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments to identify patients' emotional, physical, cognitive, and social needs, risks, and strengths.
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team and community partners.
  • Provide crisis planning and support to patients and families to promote wellness and recovery.
  • Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
  • Advocate for patients' rights and interests, and facilitate access to community resources and services.
  • Participate in education and research initiatives to advance the field of mental health social work.
Requirements
  • Master's degree in Social Work from an accredited program.
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum one year of experience as a staff Social Worker in an acute care setting.
  • Geriatric experience an asset.
  • Excellent clinical skills, interpersonal skills, and ability to work effectively in a fast-paced environment.
  • Proficient in computer applications, including MS Word, internet searches, and patient information systems.
  • Bilingualism in French/English an asset.
