Job Overview

We are seeking a highly skilled and dedicated Transitional Care Coordinator to join our team at the William Osler Health System. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions for patients throughout their hospital stay.

About the Role

The Transitional Care Coordinator is responsible for coordinating patient transfers and discharges, working closely with the inter-professional team to develop and implement effective discharge plans. This includes conducting risk assessments, identifying barriers to discharge, and collaborating with patients, families, and healthcare providers to ensure timely and safe transitions to post-acute care settings.

Key Responsibilities
  • Risk Assessment and Planning: Conduct thorough risk assessments using standardized tools to identify high-risk patients and develop targeted discharge plans.
  • Collaboration and Communication: Work closely with the inter-professional team, including physicians, nurses, and other healthcare professionals, to develop and implement discharge plans that meet each patient's unique needs.
  • Patient and Family Engagement: Engage with patients and families to understand their concerns, goals, and preferences, and provide education and support to facilitate smooth transitions to post-acute care.
  • Documentation and Reporting: Maintain accurate and up-to-date documentation of patient assessments, discharge plans, and progress towards discharge goals.

Requirements
  • Education and Experience: Bachelor's degree in a health-related field and minimum 3-5 years of experience in acute care hospital and community settings.
  • Certifications and Qualifications: Regulated healthcare professional with membership in good standing of a professional college.
  • Leadership and Collaboration Skills: Proven ability to work effectively in a fast-paced environment, collaborate with interdisciplinary teams, and lead by example to drive positive outcomes.

What We Offer
  • Competitive Salary: $85,000 - $105,000 per year, depending on experience.
