Location: Various locations across British Columbia
As a Home Health Nurse, you will provide personalized nursing care to patients in their homes, promoting recovery and enhancing their quality of life. Your role will involve assessing patients' needs, administering treatments, and supporting families in a comfortable and familiar environment.
Key Responsibilities:- Conduct comprehensive assessments of patients' health status, including physical exams, medical history reviews, and home environment evaluations.
- Develop and implement individualized care plans in collaboration with healthcare teams, tailoring interventions to meet each patient's unique needs.
- Administer medications, wound care, and other treatments as prescribed, ensuring adherence to safety and infection control protocols.
- Monitor patients' progress and responses to treatment, making adjustments to care plans as necessary and communicating findings to healthcare providers.
- Educate patients and their families about managing medical conditions, medications, and necessary lifestyle changes, providing resources and support.
- Maintain accurate and up-to-date patient records, documenting assessments, interventions, and patient outcomes.
- Coordinate care with other healthcare professionals, including physical therapists, social workers, and physicians, to ensure comprehensive support.
- Stay informed about best practices in home healthcare and changes in regulations to provide high-quality care.
- Bachelor's degree in Nursing (BSN) preferred; an Associate's degree in Nursing (ADN) may be accepted with relevant experience.
- Active nursing license in the relevant state or region.
- Previous experience in home health nursing or a related field is preferred.
- Strong clinical assessment and critical thinking skills to manage diverse patient needs in a home setting.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Ability to work independently and manage time effectively in a home health environment.
