About the Role
Vancouver Coastal Health is seeking a skilled Registered Nurse to join our Home Health team as a Complex Care Clinician. In this role, you will work with high-complex medically frail clients, providing specialized care and support to help them maintain their independence and quality of life.
Key Responsibilities
- Apply specialized theory, clinical expertise, and knowledge to address the complex medical, cognitive, and behavioral needs of eligible clients.
- Develop and implement individualized care plans that prioritize client-centered care and promote independence.
- Collaborate with interdisciplinary teams, medical providers, and community agencies to ensure seamless care coordination.
- Provide wrap-around care to designated client populations, with a strong commitment to client and family-centered care.
- Perform ongoing assessments and evaluations to identify any issues or deviations from the care plan, prompting collaboration with appropriate medical providers and care teams.
About Home Health Programs
Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative, or rehabilitative health care needs. Our clients typically have ongoing health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources.
Qualifications
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years' recent case management or home care nursing experience, including experience working with the complex medically frail population or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle. A valid BC Driver's License is required.
