Job Opportunity:

The Burlington Ontario Health Team is seeking a highly skilled and experienced Primary Care Clinical Lead to play a pivotal role in driving positive transformation and enhancing primary care in Burlington. This leadership position involves delivering exceptional guidance to enable a coordinated and connected Primary Care Network within the Burlington area.

About the Role:

The successful candidate will provide strategic guidance to support the Primary Care Network Leadership Council, establish a Terms of Reference, and serve as a proactive representative and liaison between provincial and regional committees. They will also co-develop a work plan, timelines, and metrics with Burlington Ontario Health Team leadership to establish primary care deliverables.

Key Responsibilities:

1. Leadership:

• Provide strategic guidance to enable a coordinated and connected Primary Care Network within the Burlington area

• Support the Primary Care Network Leadership Council through establishment of a Terms of Reference

• Serve as a proactive representative and liaison between provincial and regional committees

2. Improving Primary Care:

• Champion and build awareness and adoption of Integrated Care Pathways

• Support the development of team-based care for primary care providers

• Supporting the awareness and adoption of new primary care models that support access and attachment

3. Engagement, Education, and Collaboration:

• Support the Burlington Ontario Health Team Primary Care Manager with PCN membership recruitment

• Partnering with the Primary Care Manager, foster collaboration between primary and specialty care

• Strategic and Tactical: Awareness of governmental and ministerial landscape to navigate as a diplomatic and collaborative partner representing the Burlington Ontario Health Team and community on provincial clinical and primary care matters

Qualifications and Experience:

• Medical License: Holds a valid certificate of registration as a physician in good standing with the College of Physicians and Surgeons of Ontario

• Member of the College of Family Physicians of Canada is an asset

• Active Practice: Currently has a primary care practice in Burlington as a family physician

• Leadership training: Formal education in change management, leadership, or health administration considered an asset

Compensation and Terms:

The compensation for this role aligns with recommended OMA and Ontario Health Team physician leadership rates. This position operates 7.5 hours per week at a rate of $165/Hr.
