Rehabilitation Assistant - Home Health

Fraser Health is seeking a skilled Rehabilitation Assistant to join our Home Health team in Coquitlam, BC. As a key member of our team, you will work closely with therapists to implement rehabilitation treatment plans and programs in a home or community setting.

Key Responsibilities:
  • Carry out designated components of a rehabilitative treatment plan and/or program, including activities of daily living, mobility, strengthening and balance exercises/activities, and leisure therapy.
  • Observe and report cognitive and functional changes in clients' abilities or behavior to the therapist, and progress treatment plans within established parameters.
  • Document client information and complete required reports, such as workload and department statistics.
  • Assist with manual and power wheelchair mobility training, and prepare and modify equipment as needed.
  • Provide education to clients, family, and other healthcare providers on patient lifts, transfers, positioning of clients, and good body mechanics.
Requirements:
  • Completion of a Rehabilitation Assistant Program, plus one year of recent related clinical experience.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes.
