Job Summary

This is a challenging opportunity to join the Yukon government's Continuing Care team as an Admission and Assessment Coordinator. In this role, you will be responsible for assessing client referrals into Continuing Care programs, performing standardized intake assessments, and determining the appropriate level of care.

Key Responsibilities

  • Assess client referrals and determine the appropriate level of care
  • Perform standardized intake assessments and develop plans for clients with complex care needs
  • Coordinate admissions into Continuing Care programs and services
  • Provide case management and care coordination in a larger healthcare system

Requirements

  • Post-secondary degree in Social Work, Nursing, Occupational Therapy, or Physiotherapy
  • Experience working in a healthcare environment, preferably in home care and long-term care services
  • Strong assessment skills and ability to develop, implement, and evaluate plans for clients
  • Excellent interpersonal, conflict management, and communication skills
