Job Summary

This is an exciting opportunity to join the Yukon government as an Admissions / Assessment Coordinator. The selected candidate will be responsible for assessing client referrals into Continuing Care programs, performing standardized intake assessments, and coordinating admissions into the programs/services within Continuing Care.

Key Responsibilities

  • Assess client referrals and determine the appropriate level of care
  • Perform standardized intake assessments and identify client needs
  • Coordinate admissions into Continuing Care programs and services
  • Provide primary case management up to the admission date

Requirements

  • Post-secondary degree in a related field (Social Work, Nursing, Occupational Therapy, or Physiotherapy)
  • Experience working in a healthcare environment
  • Experience with provision of home care and/or long-term care services
  • RAI-HC assessment experience is an asset
