Job Summary
This is an exciting opportunity to join the Yukon government's Continuing Care team as an Admission/Assessment Coordinator. In this role, you will be responsible for assessing client referrals into Continuing Care programs, performing standardized intake assessments, and determining the appropriate level of care.
About the Role
As an Admission/Assessment Coordinator, you will work closely with the Manager of Strategic Operations, Home Care, to coordinate the admissions into the programs/services within Continuing Care. You will also be responsible for primary case management up to the admission date.
Key Responsibilities
- Assess client referrals into Continuing Care programs
- Perform standardized intake assessments
- Determine the appropriate level of care
- Coordinate admissions into the programs/services within Continuing Care
- Provide primary case management up to the admission date
Requirements
To be successful in this role, you will need to have a post-secondary degree (or diploma) as a Social Worker, Registered Nurse, Occupational Therapist, or Physiotherapist. You will also need to have experience working in a healthcare environment, experience with provision of home care and/or long-term care services, and experience doing RAI-HC assessments.
