Job Summary
Vancouver Coastal Health is seeking a skilled Social Worker 3 to provide clinical supports and consultation to programs in the local area. This role will involve traveling to home visits, hospitals, clinics, and health centers to meet with staff, physicians, and clients.
Key Responsibilities
- Provide consultation, education, and mentorship to staff on complex care planning and clinical guidance.
- Develop decision support tools and policies to improve practice and organizational compliance.
- Provide consultation and mentorship on abuse, neglect, and self-neglect, and apply knowledge of resources and intervention strategies.
About the Role
This position will have an Indigenous specific focus to build and strengthen partnerships with Indigenous Health, First Nation's Reserve Communities, and Health Centers.
Qualifications
- Master's Degree in Social Work from an accredited University.
- Post graduate continuing education training in working with vulnerable/incapable adults.
- Five (5) years' recent, related experience in working with issues of adult abuse and neglect and self-neglect.
- Current full registration with the British Columbia College of Social Workers.
