Care Coordinator for Palliative Care

BAYADA Home Health Care, a leading provider of home health care services, is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in Vermont.
This is a full-time position that offers a competitive salary of $10,000 plus a retention bonus, making the total estimated annual salary around $70,000-$80,000 depending on experience.
The ideal candidate will have prior experience in home care and/or hospice settings, with a strong focus on providing palliative care to clients. The RN will be responsible for making home visits, administering medication, wound care, treatments, and comfort care as needed.
Key qualifications include:
- A current license as a Registered Nurse (RN) in Vermont/NH
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care and/or hospice experience preferred but not required
- Solid computer skills and prior experience with electronic medical records (EMR) are a plus
