Rehabilitation Specialist

Job Summary:
The Community Support Counselor-Rehabilitation Specialist plays a vital role in our multidisciplinary team, providing rehabilitation and support services to individuals with severe and persistent mental illness. This position requires a strong understanding of psychosocial rehabilitation principles and the ability to work effectively with individuals, families, and community resources.
Key Responsibilities:
- Conduct comprehensive rehabilitation assessments and develop individualized plans to support clients in achieving their goals.
- Provide counseling and support services to clients and their families, addressing issues related to mental health, daily living skills, and community integration.
- Collaborate with the multidisciplinary team to ensure comprehensive care and support for clients.
- Develop and maintain relationships with community resources, including housing, social, financial, and vocational services.
- Stay up-to-date with best practices in psychosocial rehabilitation and apply this knowledge in daily work.
Requirements:
- Post-secondary educational preparation in vocational counseling, human services, or a related field.
- Experience working with individuals with concurrent disorders and severe and persistent mental illness.
- 3-5 years of experience in a related field, preferably inpatient and outpatient settings.
- Excellent assessment, crisis intervention, problem-solving, and organizational skills.
- Sound knowledge of the principles of psychosocial rehabilitation and the ability to apply this knowledge in daily work.
- Excellent interpersonal, communication, and organizational skills, with the ability to work effectively in a team.
- Demonstrated ability to be flexible in work assignments and to adapt to changing priorities.
- Demonstrated commitment to personal and professional development.
- Sound knowledge and understanding of systems issues, legislation, Mental Health Reform, and patient entitlements for the seriously mentally ill.
- Good family counseling skills and experience with dysfunctional family systems.
- Demonstrated best practice competencies in psychosocial rehabilitation.
- Experience in forensics, dual diagnosis, and developmental delay is an asset.
- Basic understanding of psychotropic medications.
- Knowledge of housing, social, financial, vocational, and other community resources.
- Good working knowledge of the ACT Model.
- Ability to work effectively under pressure.
- Working knowledge of MS Word and Outlook.
- Valid driver's license, adequate insurance, and access to a car.
- Sound knowledge of serious and persistent major mental illness symptoms and impact on functioning.
- Good attendance and work record are essential.
